PHOTODYNAMIC THERAPY (PDT)

Photodynamic therapy (PDT) is an exciting treatment performed with a topical photosensitizing agent called Levulan® (5-aminolevulinic acid or ALA) activated with the correct wavelength of light. This increase in the sensitivity of the skin and the target lesions has the effect of supercharging the effectiveness of the laser therapy. Photorejuvenation using intense pulsed light (IPL) and Levulan® is now the gold standard of non-ablative treatments. These treatments can not only remove sun damaged precancerous zones and spots called actinic keratoses, but also improve the appearance of fine lines and blotchy pigmentation.
In addition, Photodynamic Therapy targets the three main factors that cause acne: the overproduction of oil by enlarged oil glands in the skin; blockage of the hair follicles that release the oil; and a growth of bacteria called Propionibacterium acnes (P. acne) within the hair follicles. Once applied to the skin, the ALA is absorbed in the oil glands and hair follicles. It converts the porphoryn produced by the P. acne into a form which absorbs the light energy more effectively. When activated by the intense light or laser, the ALA reduces the oil glands, removes the P. acnes bacteria, minimizes pores, and improves the appearance of some acne scars. In summary, this treatment helps diminish, and in some cases completely resolve the acne for up to 1-2 years.
Since Rosacea is in effect a combination of an acne process as well as an abnormal increase in vascular structures such as spider veins (telangeiectasia) and diffuse redness, Photodynamic therapy is an incredibly effective treatment for this condition also. As a condition with a strong hereditary component, maintenance treatments may be necessary.
Most patients can achieve results in 2-3 treatments which otherwise would have take 3-5, or which would otherwise not have been possible for some conditions without this treatment method.
With Levulan®, however, due to the increase in sensitivity of the skin to sunlight, it becomes even more critical to avoid sun exposure for a few days after treatment. Some individuals have an exuberant response to PDT and experience marked redness and temporary swelling for a few days. Darker pigmented lesions can become temporarily even darker and then peel off leaving fresher unblemished skin underneath. |
